Community Health Needs Assessments

To help Saint Luke’s Health System improve health through outreach, prevention, education, and wellness efforts, we conduct Community Health Needs Assessments (CHNA) for each of our hospitals. We use a collaborative approach that’s designed to identify and prioritize the greatest health care needs in each of the communities we serve.

Strategies for meeting the identified needs of our communities are outlined in Community Benefit Implementation Plans, which include goals and objectives for implementing community benefits.

The CHNAs fulfill a requirement of the Affordable Care Act, and they reflect our hospitals’ longstanding commitment to partnering with our communities to ensure impactful care to the people we serve.

Community Health Needs Assessment Implementation Plans

Saint Luke’s Health System develops Community Health Needs Assessment Implementation Plans for each of our hospitals based on the results of Community Health Needs Assessments (CHNA) of the communities we serve.

Implementation Plans serve as strategies for meeting the identified needs of our communities and include goals and objectives for implementing these benefits. The plans also meet the requirements for community benefit planning as set forth by federal law, such as the Patient Protection and Affordable Care Act of 2010.

These Community Health Needs Assessment Implementation Plans reflect Saint Luke’s longstanding commitment to partnering with our communities to ensure we continue to provide impactful care to the people we serve.
