Shock
Documentation of hypotension requiring pressors cannot be interpreted as shock. Documentation of low cardiac output/index cannot be interpreted as shock.
Known, Suspected, or Likely may be documented to capture an acute episode of shock as long as the type is specified.
Types of shock include:
- Hypovolemic
- Hemorrhagic
- Septic
- Cardiogenic
Clinical indicators to support shock may include:
- Hypotension
- Tachycardia
- Heart failure
- Low cardiac index
- Respiratory failure
- Altered mental status
- Renal failure
- Hemorrhage
- Patient requiring pressors
