Short section of Central Line Catheter tip approximately 1-2 inches long removed from rest of tubing

1. Tip of catheter can be cut off and put into screw-capped sterile container maintaining sterility at all times.
2. Label the container with the patient's full name, date of birth, identification number, date and time of collection, initials of the person preparing the specimen and the specimen source.
3. Forward the container with the catheter tip in it promptly.
4. The specimen source is required on the request form for processing.
