EGFR Mutation Tumor Analysis
Lung tumor only in one of the following:
Tissue Block:
Formalin-fixed, paraffin-embedded. Tissue should be well-fixed in formalin. Ship at room temperature.
Paraffin-Embedded Tissue Slides:
Send all slides (5 minimum) within 5 - 7 days of cutting. Pre-cut slides from paraffin block in 7 micron sections.
Use non-charged, non-coated slides.
Ship at room temperature.
Fresh Snap Frozen Biopsy:
Fresh tissue should be snap frozen and stored at - 20 °C.
Ship frozen.
Plasma:
4 mL of EDTA plasma frozen (2 mL minimum). Plasma must be separated and frozen within 4 hours of collection. Send frozen. Must submit the patient's diagnosis (or ICD10 code) or testing will be delayed.
For tissue samples only:
1. Complete the Integrated Oncology request form, mark as insurance bill and send a copy of the facesheet. Include diagnosis code, physician and npi number.
2. Provide a copy of the original pathology report.
3. Label the specimen with the patient name and ID number. Place the samples inside a specimen transport bag.
4. Use small Integrated Oncology boxes and Fed Ex air bills to send directly to Integrated Oncology, Los Angeles.
Tumor sources other than lung are not acceptable.
