Collection Procedure:
1. Label the specimen with the patient's full name, date of birth, date and time of collection, and the initials of the phlebotomist.
2. Complete a "Blood Component Transfusion Order" and forward it with the specimen. Requisitions for this procedure cannot be processed unless the information requested is supplied.

Unacceptable Condition:
1. Avoid hemolysis.
2. Improperly labeled specimens cannot be accepted.
