Collection Procedure:
1. Submit mold or yeast on Sabouraud's dextrose agar slant. The organism must
be in pure culture, actively growing, placed in a proper mailing container, and labeled as an etiologic agent, if appropriate. (Agar plates are acceptable but should not be mailed.)
2. Label the container with the patient's full name, date of birth, identification number, date and time of collection, initials of the person collecting the specimen, and the specimen source.
3. The specimen source is required on the request form for processing.
