Patient should be fasting for at least 8 hours prior to specimen collection.

PATIENT'S AGE, SEX, HEIGHT AND WEIGHT ARE REQUIRED FOR TESTING.

1. SEPARATE THE SERUM FROM THE CELLS WITHIN 2 HOURS OF COLLECTION.
2. Submit separate frozen specimens for each test requested.

Unacceptable Condition:

Samples that are grossly hemolyzed, grossly lipemic, are improperly labeled or are non-fasting will be rejected.
