Collection Procedure:

Sample collection (6-8 inches of Umbilical Cord) will be done by nursing.

Nursing will complete Chain of Custody (COC) form. (NO signatures are needed from laboratory!)

Nursing will place correctly labeled sample container with white COC form in box (nursing will retain pink copy and forward yellow copy to lab along with boxed sample). Box will be labeled with Beaker label.

Per USDTL, if white COC form is located outside of the box, they will still accept specimen. Keep box bagged with white COC form.

 

Specimen Processing:

Lab will use Beaker label on outside of box to receive testing and also to place on a packing list.

SLE, SLN and Hedrick will send boxed samples and "yellow" copy of COC form to SLH on a universal packing list. (NO signatures from Lab or courier needed on USDTL COC.)

Printed packing list for USDTL should NOT be sent to USDTL (United States Drug Testing Laboratories) but retained in Reference Lab. "Yellow" copy of COC form will also be retained in Reference Lab.

FedEx labels and shipping bags are in Reference Lab.

Saturday FedEx pick up will need to be scheduled by Plaza inpatient processing if there are samples to be sent. FedEx must be contacted by noon. See "LAB PERSONNEL ONLY" section below--must be signed in!

Unacceptable Condition:

Bloody specimens, specimens fixed with formalin, obvious decomposition, tamper-evident security seal absent or not intact, chain-of-custody documentation incomplete. 

Do not use ethanol containing products in or around the collection area when collecting specimens for ethanol or ethanol metabolite (such as ethyl glucuronide, ethyl sulfate, or phosphatidylethanol) analysis.

Initial screen and confirmation. All specimens are initially tested using Enzyme-linked
Immunosorbent Assay (ELISA), Laser Diode Thermal Desorption Tandem Mass Spectrometry
(LDTD-MS/MS), or Liquid Chromatography Tandem Mass Spectrometry (LC/MS/MS) and nonnegative specimens are reflexed to LC/MS/MS or 2-Dimensional Gas Chromatography-Tandem
Mass Spectrometry (GC/GC/MS/MS) for confirmation testing.
